Vascular disease is a broad term encompassing any condition that affects the circulatory system, including arteries, veins, and lymphatics. Abdominal Aortic Aneurysms (AAA)

The aorta is the body’s primary artery, originating from the heart and descending through the abdomen. An Abdominal Aortic Aneurysm (AAA) represents a pathological dilation of the aortic wall, causing it to bulge outward. The clinical danger of an AAA lies in its asymptomatic nature in the early stages. If the aneurysm expands beyond a critical diameter, the risk of rupture increases significantly, leading to catastrophic internal hemorrhage.For a screening of an AAA, the most important demographic is male patients aged 65 years and older who have a history of smoking.

Aortoiliac Occlusive Disease

This pathology involves the occlusion of the distal aorta and the iliac arteries, which perfuse the lower extremities. For patients with end-stage renal disease, hemodialysis is a life-sustaining treatment. To make dialysis efficient, surgeons must create Hemodialysis Access, typically an arteriovenous (AV) fistula or graft. This procedure connects an artery to a vein, allowing the vein to handle the high blood flow required for the dialysis machine to filter the blood effectively.

Iliofemoral Deep Vein Thrombosis (DVT)

Deep Vein Thrombosis (DVT) is the formation of a thrombus within the deep venous system. Iliofemoral DVT specifically localizes to the iliac or femoral veins of the pelvis and thigh. This condition constitutes a medical emergency due to the high risk of pulmonary embolism (PE), where the thrombus detaches and obstructs the pulmonary vasculature. Clinical presentation often involves significant edema, erythema, and pain in the affected extremity.

Mesenteric Ischemia

This condition affects the small intestine. When the arteries supplying the bowel become narrowed or blocked, patients may develop Mesenteric Ischemia. Chronic mesenteric ischemia typically manifests as postprandial angina (severe abdominal pain 15 to 60 minutes after eating), leading to sitophobia (fear of food) and unintentional weight loss. Acute mesenteric ischemia, often embolic in origin, presents as sudden, disproportionate abdominal pain and requires emergent surgical revascularization to prevent bowel necrosis.

Peripheral Arterial Disease (PAD)

PAD is a systemic manifestation of atherosclerosis affecting the extremities. The accumulation of fatty deposits narrows the peripheral arteries, reducing distal blood flow. The hallmark symptom is intermittent claudication, muscle pain during exertion that resolves with rest. Without management, PAD can progress to Critical Limb Ischemia (CLI), increasing the risk of infection and potentially leading to gangrene and tissue loss.

Renovascular Disease (RVD)

Renovascular disease affects the renal arteries that supply the kidneys. Stenosis of these vessels triggers a physiological response where the kidneys release renin, drastically elevating systemic blood pressure. Consequently, RVD is a leading cause of secondary hypertension. Chronic hypoperfusion may eventually lead to ischemic nephropathy and renal failure if arterial patency is not restored.

Thoracic Outlet Syndrome

Thoracic Outlet Syndrome (TOS) is a group of disorders that occur when blood vessels or nerves in the thoracic outlet, the space between the clavicle and the first rib, are compressed. Etiologies include physical trauma, repetitive stress injuries, or anatomical anomalies. Symptoms are variable but frequently include neurogenic pain in the neck and shoulder, along with paresthesia or weakness in the distal upper extremity.

Non-Salvageable Extremity (Amputation)

Revascularization procedures are the first stage of intervention to save the limb, aimed at restoring blood flow whenever possible. However, in severe cases of infection or tissue death, a Non-Salvageable Extremity may require amputation to save the patient’s life. At Vegas Vascular, we consider amputation as the last resort. If we are required to perform the procedure, we then attempt to shift the focus of the surgery to the rehabilitation of the limb so that the patient can achieve ambulation with a prosthesis.
